  1. Download Form W-7 (October 2024 revision or later) from irs.gov.
  2. Check the “Renew an existing ITIN” box at the top of page 1.
  3. Enter the prior ITIN on line 6e. If the prior name has changed (for example, after marriage), enter the previous name on line 6f.
  4. Complete the remaining personal-information fields (name, address, date of birth, country of citizenship).
